Please bring your insurance card to each office visit. If you do not have a current insurance card and we are unable to verify insurance converage, we require payment at the time of the visit. Please notify our office staff of any changes to your registration information. It is your responsibility to update your information to our office.

We are contracted with Mills-Peninsula Medical Group and numerous other insurance plans. Please call our Billing Office, MD Billing at (650) 839-2500 if you have questions about your specific insurance. Beginning December 11, 2006, our new Billing Office number will be (650) 288-0600.
